Not so much dying as having no right to still be alive.

Fighting a warlock murloc deck. I manage to get board control and have lethal next turn, but I'm at 8 health. He's got 3 cards in his hand and it's his play.
Casts Soulfire. Discards the other Soulfire. Concedes.
By all rights that should've killed me.

Hearthstone.

He didn't. Soulfire (and any other effects that causes you to discard cards) always discards a card at random, you never get to choose which card to get rid of.

You know the DayZ effect? When you are allowed to be an asshole, with no real consequences, you will. Playing a server where you get banned for 3 days if you die, and I'm sitting around outside spawn, talking to everyone, when some jerk walks up and one shots me. My gear is more then worthless to him, since all it does for him is fill up his inventory. I need to get a life before I shoot someone over this or something.

After a rather nasty ordeal with two Archers and a narrow walkway, and a statue with an incredible amount of health. I encountered Jay and Silent Bob, and despite my relatively nice gear they still kicked my ass six ways to Sunday multiple times.

Playing as Snake against a Meta Knight, Samus and Wolf on a modded Final Destination.
Slipped over the left edge while doing Snake's Air Side attack and didn't have time to use the Up Special.
Last life, too.

I'll just take a guess and say Super Smash Bros Brawl (or one of the SSB games, anyways, don't know them, despite having Nintendo consoles... Probs should work on fixing that, hmm...)

Given it including Snake, Meta Knight, or Wolf, it has to be Brawl. (Or Project M, since it was a modded Final Destination, but that's just Brawl with fan changes.)

SUPER LATE EDIT TO AVOID DOUBLE-POST: A 'should've died' situation that I would've felt guilty posting in the 'own' thread. My control Mage had been behind against a control Priest for pretty much the entire match, with me surviving on top-decks more than once. After many spells and nukes from both sides, I'm down to 9 HP, no cards in hand, and only a Mirror Entity in play. He's got 2 cards in hand, 16 life, and it's his turn. I've only got 5 cards left, of which one is Polymorph and one is Counterspell, meaning I've got almost no damage left in my deck. If he doesn't kill me soon, fatigue will destroy me.

He plays Deathwing. On an empty board.

Mirror Entity copies Deathwing.

I top-deck Polymorph (out of 5 cards left, so only mostly luck).

This, ladies and gentlemen, is why you don't play Deathwing into a Mage with an active secret. Especially when that Mage played the other Mirror Entity earlier in the match.

Dammit Elodie, who the hell told you the best way to treat an arrow wound was to shove it in further. Hell, the attempt to pull it out was a bad enough idea.

Prolly trying to push it out the other side so the head could be cut and the shaft safely pulled out, which is only a good idea if its already more or less poking all the way through you.
