I stepped away from a guard who was trying to search me at the entrance of a rehab clinic. Cue five guards running in and filling me with lead.

One-shotted by a freaking rainbow while under the Zodiac Curse.

Before that, mobbed by blue Pfhor fighters at close range. Teleporting ambushes in narrow passages are cheap.

Got lost in some random kwama egg mine, met some random guys in the darkness, and got killed by them after pretty lengthy chase. (Morrowind, using mod which makes dungeons and nights pretty dark without light source.)

Was throwing a firecracker, but it blew in my hand so I took damage from it and I also flew to another square where I broke my leg and then bled to death.

I built a nuclear reactor right next to my other "appliances"  :P
(Minecraft, modded with IndustrialCraft)

My cities accidentally started making nuclear toys, my enemies attacked because of that, then they got pwn'd, then they blew up all my cities. :'(
