Hardcore Space Marine: start as Imperial Guardsmen on a planet beset by every faction except the Space Marines. They arrive at the end... to set up the markers for Exterminatus.
Hardcore Surgeon Simulator 2013: You're operating on a dude to turn him into a space marine.
I'd play it.

It would be a hell of an operation, considering turning someone into a Space Marine involves around a dozen organ insertions.

I think they split it into multiple operations. The otherwise-terrible Blood Ravens novel series takes that viewpoint. Of course, the unlucky bastard is left strapped to the operating table in between operations and they don't exactly bother with proper sterilization...

Right, exactly. They also don't bother to sew initiates up after each surgery.

Huh, this is sounding more and more like Surgery Simulator 2013 the more we talk about it. Make it happen!

Huh? MSH is right; the operating room is filled with toxins and alien plagues in order to test initiates. If they can't survive the surgeries, there's no way they can handle real Space Marine life.

Hardcore Bioware RPG: All romance characters have STDs. Also, failing their dialogue tree means that they instantly kill you in a broken-hearted rage.
Hardcore Any RPG: Everyone is Yandere, everyone wants in your pants, everyone will fucking kill you and each other for the crime of being wanted by anyone but them.
To survive you need to play them off each other until only one remains.  Kind of like they're Highlanders, except the Prize is you.

And a sane amount of ammo. And cars cant be just driven off, oh, and everyone has a phone to call the (overly-armed) cops with.

I did once come up with an idea for GTAalike that would feature an in-depth (AKA more Liberal Crime Squad, less Just Cause 2) depiction of running a revolution against an oppressive government figure, but that'd be more in line with the Games You Wish Existed thread rather then this.
