I promptly had to run from about fifty Dread Collectors. I did not survive.

Kerbal Space Program

I was hunting serial killers in Gangland when I got one!  I started fighting him, then to my horror, I realized he had a fire axe.  A few blunt chops across my head and body through my HEV later, my entire ribcage is shattered and I'm left to bleed on the floor while my corp comes to kick his ass and loot him.

Hatemoo.  I was unlucky there, it was a serial killer boss instead of a normal one and he had buffed stats to match.

Turns out babies, and Child Protective Services agents for that matter, are surpisingly more flame-retardant than I originally thought they were. The mother, on the other hand... not so much.
And the mother had been starved, sleep-deprived, etc. for some time while she kept poking that fireplace, setting off the inferno. First, the other parent died, and she watched the Grim Reaper come for them, who himself was slowly driven insane by half a dozen crying babies. Then, as the flames consumed everything, the last thing her poor, tortured frame saw was CPS teleporting he kids to safety while the worker, first shocked at the flaming property, took one look at her, then left with what I swear was a smirk on her(the worker's) face.

And then the Grim Reaper came a second time. Noticably relieved that there were no more crying babies.

I'm going to hell for this for sure.

The Sims 3. Oh, and CPS was there not because the attempt at flaming babies, but because of starvation. It took a damn long time to start that fire. Turns out I was using the wrong fireplace. The cheapest one doesn't start fires by poking.

Demarius-3 rocket, because of my miscalculations, failed to make a full circle around the planet. Fine, I said, and so I let it descend into the atmosphere, until it reached the altitude of ~30.000 metres.

At said altitude, I hit spacebar to separate the prime rocket from the command center, and to my horror, they didn't separated at all! Only then I understood that the rocket is falling at totally wrong angle, velocity and gravity keeping the two together. Quickly turning off auto-SAS, I began trying to spin the module so it would 'slip' away from rocket and it's buddy, the force of gravity.

At last, I managed to get it off! Now, just launch the parachute and-*KA-SLAM*

I was clearing out a demonic asylum. When suddenly, I saw a cluster of... err... I'm going to call them zombies. I pulled out my stake-gun and launched a grenade with it, but a second before I pulled the trigger, a zombie lept foward into the grenade, making it explode. The blast killed me.

Painkiller. A great game.
