The nice clean cavern water contains the poop of your local antmen tribe, of the elk birds, of the pond grabbers, and of the multitude of forgotten beasts that roam around on your world.

At one point, I had a FB that was somehow oozing all the time. It liked to swim around in the water, and believe me, that wasn't "nice and clean" anymore very very quickly.

Alcohol can kill bacteria or other nasty things, but only if the concentration is high enough. The amount of alcohol required to do this is lethal if your blood was made up of the same percentage of alcohol. This is why way back in the medieval era no one drank water from a well. Doing so would kill you. Instead everyone drank booze, with drinking from a well as absolute last resort.

The alcohol in the booze would kill the microorganisms, making the liquid safe to drink.
...These are DWARVES. They drink alcohol without becoming dehydated. I don't think a little booze blood would hurt them.

My hospital is a death senntance, not because of disease, but because of lack of water.

All the ponds evaporate before I can get water into a cistern.

Give cats natural metallic armor and throw them in your danger room.  Also allow their mouth and tail to grasp (shield in mouth, weapon in tail xD)  Have a cat based military.  You know, do the same with all tame animals xD send in the cats as shock troops to disrupt the archers

Short answer: Make soap and put in hospital by adding storage containers to the hospital. Solution is only successful some of the time.

I'll have a banana please.

So far, I've got a 100% survival rate in my hospital; the key thing seems to be making sure sutures/bandages are applied ASAP and there's soap on hand.  Infections don't seem to matter after that.

You're lucky, I have a random idiot at the well who keeps "applying plaster"... to patients who are lying in the hospital, next room. He keeps doing whatever he's doing without moving an inch, and he's thirsty. Deconstructing the well won't do anything. I should just try digging a channel under him before he dies of thirst in front of the well. And then kill him for being an insufferable moron.

That's tricky, but hospital stockpiling zones don't automatically bring gypsum powder to them. And even if they did, they'd have every allocated storage overloaded with cloth and thread of every quality before it even arrived (above and beyond the inventory limits given to them, the thieving bastards!) You have to set up the stockpile within the zone manually for bags (no sandbags!) with material gypsum powder. When it's there, they'll use it.

But aside; that won't do anything for infections.

Thanks, I'll get some powder moved right away.  And yeah I know it doesn't do anything for infections themselves but maybe if the guy gets his leg set instead of just laying there with a compound fracture for 6 months he won't get an infection anyway.

How do you make it "ASAP" though? My medical dwarves always take a while to get to work, even with all other labors turned off. They sit around while the guy is trying not to bleed to death, then they eventually go over and do one procedure. Then sit around with "no job" some more and do another, and so on and so forth.

Don't get me wrong, eventually, they do everything. It just takes a pretty long time, even though I have 4 dwarves (Chief med dwarf, one surgeon, one suturer, one wound dress/bone doc). Last time I had a big siege with 4-5 guys getting injured and the last 2 ended up getting infections.

One of the dwarfs walked in front of Thor to get a better view of the prye, and Thor kicked him irritably into the middle of the flames, which made Thor feel slightly better and made all the dwarfs feel much worse.
