Oh, you dwarves. Whining that you need soap for cleaning an infection is just brilliant.

They've barrels of a really good antiseptic lying around: some high-percentage booze which would kill off any bacteriae quite nicely, but noooo... must clean with some globs of a mixture of fat and lye which probably doesn't kill these little critters at all.

Unless the infection is already gone into the lymphatic system or something. But then the soap should be just as useful as pouring some booze over it would be (i.e. not useful at all.)

I have lost one dwarf to an infection so far. He wrestled with a groundhog, his arm got hurt, and I hadn't a hospital yet (because I had no watersource anyway.) He ran around and worked, and at some day, he just succumbed to the infection while carrying an orthoclass block through the corridors.

I like to put burial grounds next to hospitals, saves on walking.

If I recall may biology courses correctly, in real life, alcohol only stops bacteria proliferation for a while, while soap really destroys bacteria.

Don't quote me on that.
Cacame channels aquifers into submission by staring at them. Cacame is so badass, kobolds give him their children to leave them alone. If Hidden Fun Stuff digs too far down, they hit Cacame. Cacame once took a Tantrum Spiral and impaled four enemies on it.

Alcohol can kill bacteria or other nasty things, but only if the concentration is high enough. The amount of alcohol required to do this is lethal if your blood was made up of the same percentage of alcohol. This is why way back in the medieval era no one drank water from a well. Doing so would kill you. Instead everyone drank booze, with drinking from a well as absolute last resort.

The alcohol in the booze would kill the microorganisms, making the liquid safe to drink.

Now that I think about it, why do we even need soap for medical purposes in this game? Isn't soaps primary purpose is for cleaning dirt etc. over it's antiseptic uses(could be wrong about that, anyone know more about this than I do)? Alcohol would probably be superior for antiseptic purposes, and dwarves don't seem to mind being covered in puke and blood.

What if when alcohol eventually gains liquid properties, we can add a tiny amount of alcohol to water to sterilize it? IIRC the History Channel said something about people drinking that since straight water would get you sick.

Then we could engineer waterfalls with alcohol mist, and put it in the hall to the infirmary! Passing through it could generate a happy thought, and also sterilize a patient's wounds before the operation.

That's the point, it wouldn't be as fun to crush their souls if they didn't have souls to begin with.

My last fort saw four die on the table in no time, but I wonder how much of that might be due to the Terrifying land. This fort is in a warm, mildly savage land, and so far no infection deaths. One dwarf has lived with infection up and down his leg for about 3 years with no ill effect.

Don't use pond scum water ever in your hospitals. I think it might be better to skip the washing step then wash with pond scum water.

I've never had an infection (that I've noticed anyway) using cavern water (either straight from the cavern or pumped into a reservoir). Pond scum water seems to be about a 50%+ infection rate.

I never use soap, mostly because I can't be bothered to make it. Using soap might let you use pond scum water but really I wouldn't risk it since every map has nice clean cavern water to use. It's also (usually) safer to get cavern water then risk having your doctor get ambushed by goblins or eaten by tigers trying to gather a bucket of pond scum water.

Edit: as for the actual question, I don't think you can get rid of infections. The dwarf just dies eventually. If they have a high disease resistance they might get over it on their own but I've never seen it happen.

I had a deer escape from a cage trap and hack up the fool carrying the thing...smashed his left hand and leg completely open.

He got hit with infection on both sites, I built a hospital maybe 2 months later and patched him up with a guy using 0 skills, muddy pool water and no soap...he seems over it.  Halfway through his convalesce I built a nice well, didnt notice till after he came round that it was salty.

Seems there really is some random element at work here

Best solution:
Mod out infections for dwarves.

Now if only I could figure out how to do that without disabling all syndromes.

Funny, just after reading this I saw a bone doctor cleaning some vomit out of the hospital while hauling a barrel of wine.

A fully stocked hospital has saved most of my adults from dying of infection. The children and animals are dropping like flies. Forgotten Beast Extract that causes heads to rot = Fun.

I've got two legendary axedwarves that are running around right now having no apparent difficulty despite having guts that are suffering from "advanced rot" due to a forgotten beast's deadly dust.

Which is actually pretty horrifying when I think about it. Mighty adonises of dwarfdom, stomping around in their adamantine plate, unstoppable killing machines... rotting undetectably on the inside. One day they'll probably get hit by a goblin and foul-smelling corrupted goo will burst out of the wound. Eugh.

wait, soap WORKS now?
No more going to the soap, grabbing it and then dropping it with a weird error?

It's probably made from baby bone, with a handle of baby leather. Probably uses the leg bones wound together for the handle, the pelvis for the handle/pick joint, and the pick is the spine.

But that's all in theory, of course. Not like I've made a pick out of my own 5 month old baby before.

That still happens. Keep all soap in hospitals.

I'm somewhat terrified what will happen when diseases get implemented.

A lot of FUN will happen.
