Add explosive diarrhea as a syndrome effect. It causes dwarves to occasionally generate massive amounts of miasma and magma mist. If it's above 100 strength it causes the dwarf to explode when it ends.
The parts if the exploded dwarves are stuck to the walls, floor and ceilings no via blood. If I cleaned within a day, they will be stuck there forever

Dwarves are now a hive-mind. All dwarves in a civ have the same skills, personality, thoughts, etc.
If one goes insane, they all go insane.

FBs are tameable but every single job related to them gets randomly changed to a different job related to them once activated. This means that training the FB can be changed to letting it out of it's cage, butchering it, pasturing it in a random zone etc. This "curse" spreads to everything affected by it, therefore a workshop in which a FB was milked might start producing "kill Urist McMacelord" tasks, and the macelord in question will start picking random people to kill once they interact with a dorf sent to assasinate them by an evil farmer's workshop. Handling an "affected" item means being affected.

I think this is at least the fifth or sixth "explosive diarrhea" suggestion in this thread. :P

It's actually pretty terrifying to think about having all of your fat melt off into grease because you started sweating too much.

Some insane dwarves will begin to hallucinate, attacking things that don’t exist

What the hell part of this is a bad idea? I'd rather them punch down imaginary workshops than my real ones.

Also, doctors play pranks on dwarves and sew stone crafts into them.

Random meteors will fall on the surface each month, each one with minerals not found anywhere on the planet, as such they cannot be mined

Human civilizations get sacred animals during worldgen and their merchants react to anything made with any part of that animal the way elves react to wooden bins.

And unless you go through legends mode before starting your fort the only way to find out what their sacred animals are is to piss off their merchants.  Armok help you if you're in trading range of two human civilizations with different sacred animals.
