Hardcore Disease-mode: Every organism has 99 Constitution. Good luck getting past that roll.

Ahh, the RNG hates me enough as it is.

If you are a supernatural disease it is possible to attack their Will saves instead.

Hardcore Team Fortress 2: Spies can disguise at hats. The UI for that would be on par if not harder than that of Dwarf Fortress.

Yeah, this would be terrible.
TF2 already has quite the competitive scene by itself. Even the most casual games have some mad skill and hard competitive gameplay in the higher levels (e.g. Prestige in CoD. Never reached this myself, but I have a friend that has, and I've heard a lot of stories.)

Why?  TF2 is casual, sure, but its casual in a good way.  You can accomplish at least a little even if you 100% suck, but it has enough depth and skill that longtime players still dominate.  Making weapons more deadly and long-ranged would just make health less important and thus make the classes less distinct.  Not to mention kill game balance, and make the passageways most TF2 maps favor into impassible projectile-laden chokepoints.

I ended up at about 6th prestige in World At War. Nothing changed for me, gameplay-wise.

To me casualness and hardcoreness isn't necessarily a sign of how competitive a game is (The "Pros" of Super Smash Bros really should learn that).

It just determines how soft handed it is.
