How I can choose a quality level for a uniform item?

The option to equip specific items is right there if you actually look in the military screen, but it can't be applied at the uniform level, only to each individual dwarf (hence the game's use of the word "specific" in this context). It's highly inconvenient so I wouldn't recommend it: specific items you assigned one guy don't disappear from the list you see when equipping the next guy.

Otherwise, dwarves will try to equip the highest quality items they can get, with squad members higher on the list getting first dibs.

Most expensive, not necessarily highest quality. It is perfectly possible for dwarves to choose that candy encrusted low quality wooden sword over a masterpiece steel one.

Since I'm not particularly big on decorating, at least not in my current fort, I never noticed.
