The AO rating is for pornographic games. Like, that's what it's for. I think there may have been one game that got it on sheer violence alone, but the game was a shitty unreleased PS1 mortal kombat ripoff whose title I can't remember anyway and whose rating may be apocryphal.

From a brief glance: Manhunt, Thrill Kill, and GTA: San Andreas.

San Andreas got slapped with an Ao rating for the nulled-out sexual "Hot Coffee" minigame that hackers managed to reactivate, not violence.

Manhunt 2 was slapped with an AO for a playable POV necrophilia scene.
