Attack or where it hits?

Attack. It's pretty far. Hit locations are not affected by modifiers.

A called shot means you aim at one particular part, losing the randomness of the location roll, but also taking a penalty to your attack roll, because it's harder to aim at a specific part than it is to just spray bullets in the guy's direction.

So in this case, your called shot rolls would take penalties, but if they hit, they unerringly hit the part you aimed for. So the rolls are attack, defensive, and damage, skipping the location step.

Also, I'm not sure if this was clear in the rules, but called shots can be made with anything, not just ranged weapons, so in theory the three of you near the Big Man Thing could, I dunno, called-shot him in between the legs (Lower Torso)

That was clear, but as of now most enemy were vulnerable pretty much anywhere, so it was not that necessary.

Josephus, while the turn was mostly errorless, a certain phrase's wording wasn't perfect. Specifically, Cassandra's turn. She might know or feel the doors and piles of bodies being there, but she can't see them. ;)
