Story Setup:
1.) Start Setup
A.) NOT in a tavern, because it's overdone.
B.) NOT too much organizational power (leader, ruler, manager, person in charge, etc).
C.) USUALLY not yet a member of an organization (that can be earned later).
D.) Classic either start in Training, or Distress (escaping captivity etc). Starts with action, gets system in use immediately.
World Organization (General Points):
A.) Start in or near acceptable main player species Borderlands, between two or more countries, or territories or, political subdivisions (multiple sources, less supervision, more adventure).
B.) "Need Filling" Focus (Ours and Population). Makes sense, useful for adventure hooks. Can be general without specific numbers, or with. Easier without.
C.) Remember Weather. It rains surprisingly little in these games for some reason.
D.) Consider tribes / politics for enemies (goblin, orc, etc). It's a nice flavor feature.
E.) Humans, and humanoids can be bastards too. Hopefully not all of them, but yeah. No worse foe than the former friend.
Individual Player Control:
A.) Keep it to direct control over one character. NPCs can increase "party size" if you like. Important for absences in an SG. Can suggest for NPC actions though.
B.) Consider Party Role(s). If you have multiple characters or NPCs acting together, consider what role each being plays.
C.) Have a mix of linear and site based plots (as much as open world is neat, it isn't simple. Don't railroad, but keep reasonable expectations in mind).
D.) Options for individual combat rounds beyond "hit him." We've covered some of this previously.
