Just bites and scratches and it isn't a 100% chance.

If memory serves, scratching doesn't deliver syndromes, but bites that break the skin at all generally will (that is, the actual bite - I haven't seen a shake transfer the syndrome, but I could be wrong.)

You must bite to shake.

What I mean is a bite could still latch on, but not tear the skin, while the resultant shake does usually break the skin in a big way.
