I had this exact problem just yesterday. 3 dwarves died of starvation in my hospital before I realised I didn't have any "nurses" and the three doctors I had assigned were the ones that were injured. Bringing food and drink is lower priority that pretty much anything else so if there are no idlers then they will simply be left to starve which is ridiculous. They should be high priority jobs as they are infrequent and important! But regardless have some dwarves who are non-haulers with feed and drink enabled.

1x2 stockpiles avoid the problem with barrels just as well as 2x2 or larger stockpiles.  The main reason I use 1x2 as the minimum size is so that you can see whether the stockpile is full or not, without being fooled by the (usually) empty barrel in the first slot.

The lye/milk bucket bug was fixed in May.

Its not been mentioned yet but its probable that food stockpiles LESS than a 2 x 2 size will not work properly. Dwarves refuse to fill them up, instead leaving empty barrel(s) but not putting any food or drink there. I have been wrestling with this bug since back in 40d.
1x1 stockpiles work fine so long as there are no containers. Drinks are a little harder since they have to be barrelled, but I got around this by putting one stockpile over the whole prison area and then removing individual tiles until I had something that looked like a bunch of 1x1 stockpiles but was one single stockpile. You could also use multi-level jail cells and reserve one floor for booze and another for food. While you can have one stockpile giving to multiple others now, it's still quicker to have one pile split up across rooms, especially if it's a lot of keypresses to get between the feeder stockpile and the prison stockpiles.

Use the 3x3 jail, but place the chain at the back wall and make the food stockpile a 2x3 size. Your dwarf cant reach all of the drinks, but since rearranging my jail the stockpiles are now properly maintained.
Is that 2x3 stockpile one that combines food and drink? What happens if it get filled with all food or all drink?

As for why the dwarves dont feed prisoners, its always been quite iffy but it may have something to do with urist mcwoodchopper taking the job from half way across the map, deciding he is too hungry to continue when he finally gets to the hopsital cancelling it, and then urist mcminer deciding to take up the task from the bowels of the earth, causing the same thing to happen ad infiniteum.
This was fixed in 2010, when dwarves started completing their current jobs before going for food/drink/sleep. The problem is getting them to take the job in the first place.

Try as I might I could not get smaller stockpiles to work, even using the method you suggested. Im not sure why, maybe I missed something. Anyway at the moment I only keep booze in, since it seems dwarves die from dehydration faster than they do from no food. Not sure if thats the case but its worked ok so far.

Smaller stockpiles? It's still considered one stockpile even when none of its tiles are connected. You also have to order the stockpile to take from somewhere else, unless you don't stockpile booze. That actually works pretty well, since the still works slowly when there's a lot of alcohol and quickly when there's little left.

I am trying to make chickens lay bees as eggs. So far it only produces a single "Tame Small Creature" when a hen lays bees.
Honestly at the time, I didn't see what could go wrong with crowding 80 military Dwarves into a small room with a necromancer for the purpose of making bacon.

As mentioned feeding/watering is a low priority job.  However nobody has mentioned that its a job you can select for in your dwarves traits.

Some dwarves like helping others more, they even gain happy thoughts from doing so.  I always have a few of these guys lounging around as nurses, they only feed/water and rescue (and are ironclad for the third task)
