Sounds like she made a royal mess of that one...

One way to avoid having this happen is to channel out a chute above your atom smasher (which you should have enclosed by walls) and assign it as the garbage dump, that way dwarves drop garbage down a z level into your enclosed atom smasher, and you need never worry about dwarven garbage enthusiasts investigating at the wrong time.  :P

I do the same. A 1x1 pit from the top of your fort to magma flows may be harder to build (lots of digging), but easier to take care of and is less dangerous.

Dwarves dodging into the pit are exceedingly rare as well.
