It's your wealth. There is nothing wrong with your DF, you just aren't good at dwarf economics yet. It's understandable; my second fort ran for 10 years and only made ~4 million in wealth. Now on my 6th major endeavor, I have 11 million after 5 years. I have yet to lose a fort, but I'm sure something crazy will happen eventually.
Goblins will hit you at the beginning-middle of every season once you get good at making an economy. Orcs require a created wealth of 100k in a season to send the first siege, so if you run a fort for 14 years with 1.32 million you may never see an orc siege. If you want to be sure of a good siege, murder the human caravan. They come full-strength for the sake of revenge, not caring about your created wealth or anything like it. The aforementioned 2nd fort suffered the death of a human diplomat, and my 47 dwarf fortress saw a 50 human siege. Bear in mind I had ~10 children. So, like I said, the humans are guaranteed to give you a fun time.
Once you learn to control the artifact side of Dwarf Fortress and prepare for artifacts carefully, you won't see anything worth less than 100k. Sometimes you're really, really unlucky and some dwarf wants to make an item out of 2 stone or a single piece of pig tail cloth. Even then you can give them at least some platinum nuggets and a dyed cloth worth around 300.
If they use a lot of materials, it's much easier to get things worth over 1 million each.
