Possessed dwarves, usually.

Even if it wasn't ever programmed to happen, it would still sometimes happen since names for dwarves and artifacts draw from the same limited pool of words.

This is consistently repeatable, actually: if you forbid the first item tasked for the artifact while the dwarf is still gathering materials, it will gather a different object to replace it and the resulting artifact will be named after the dwarf, regardless of what the name originally would have been. Thus you'll often see this happen when selectively forbidding and claiming materials to maximize artifact value.

Children you rescue shouldn't behave like rabid beasts.  I guess your regular companions shouldn't act like rabid beasts either.
I think that's a little more impossible than I'm likely to have time for.

when she gets too smug about it, treat her the same way as one would treat a noble.

I like the thought of a kid brandishing some hideous weapon made from poultry bones and going: "I call it: Me!"

I didn't do that, though -- if I were going for maximum value, would I have chosen goose bone? (I did forbid all my cotton candy reserves, as I always do when a non-(weapon|armor)smith mood starts up)
