Then a few months later, the volcano started to overflow for some freak reason.

hmm... i've had this glitch once before. though it could be RP'd as a volcanic eruption, i think it would end with the entire surface  covered with magma.

I did not fully investigate where the caverns breach was.  I can verify that there is one due to cavern plants growing in soil, and I know there was

Based on what I did see it appears as if someone cut holes to give dwarves access directly to the volcano, rather than just remove one of the 20 or so magma workshops for a garbage pit.  Not a big deal normally, but it was a really wide hole, completely open to the volcano. When the volcano started erupting, the lava poured straight into the fort.

I did see that there was a tunnel leading down around the edge of the volcano.  Never did get a chance to investigate it.

If you do want to perform a recovery, I'd suggest enabling mason skills on every dwarf and quickly plugging the holes around the volcano caldera that lead into the fort, then keep your dwarves close to the drawbridge doing cleanup until Colossus shows up, then mobilize your troops on the drawbridge and hope the troops make him dodge into the pit.  That's how I captured it twice.

This is all likely unnecessary information though, since you are actively seeking out an ugly save 

You will love the mineral content of this map.  It's quite simply absurd.

Yup dyg did the big hole for a garbage dump, I played before and after him, I did the path around the volcano which leads to the caverns... gonna be a lot of obsidian down there soon.

way ahead of you, I used pumps to channel the river into the volcano, then shut them off when they weren't needed.

EDIT: DAMN IT! THE MILITIA COMMANDER IS TRAPPED UNDER A BRIDGE! I KNEW I SHOULDN'T HAVE PULLED ALL OF THEM!
EDIT2: He's good now; still no bronze colossus. though I did get a message saying something about an item being misplaced. I have the game window open I dou- AND THERE'S THE AMBUSH ANNOUNCEMENT! WISH ME LUCK!

The two space doorway into the volcano was me, though I'm fairly sure I put doors there. Though knowing myself I probably constructed the doors, forgot what they were meant for, and used them on a bedroom somewhere. Anything heading to the caverns was Ashton.
