This looks like the natural consequence of not letting rivers burst their banks from time to time. This sort of thing happens on the Yangtze at places thanks to many hundreds of years of flood prevention. I expect the world gen routines just don't to flooding.

Getting rivers to erode and accrete right in world generation is a pretty painful task. I don't blame Toady for imposing river channels in this way.

Could anyone tell me how it worked in 2D? I seem to recall from the Urist saga that rivers did flood in that, at least in game time.

Back in the times of old, when dwarfes could not look up in the sky...I remember underground river flooding every spring.
Do not know about aboveground rivers thou.
