This sounds like a bug to me if it happens too often. I use stairs quite a lot, and I have not seen this before.

Can we get a save? I wonder what kind of fall can tear your limbs off. Maybe it was a cave-in.

I've had dwarves give birth on stairs without any endlessly falling and exploding babies. Also, I have very very very high stair blocks (sometimes, they go from surface to magma sea), and yet no injuries on or under the stairs. So yes, I think you have identified a bug.

So this staircase goes all the way to the surface? If a dwarf gets sun-sick while trying to use the stairs he can pass out then fall all the way down? Hilarious.

Good thing I don't do the big central staircase connecting everything design.

Are you sure the death is caused by a fall, and, if so, that it is the dwarf that is falling?
Getting run over by a mine cart can be hazardous to your health (a few cats have paid for their curiosity that way). Also, I assume there is only a staircase, and not open space on any side of it?

Floor hatches stop dwarves from falling down stairs. I put hatches on all my stairs, because this has been an issue for a long time. It has to do with how dwarves make "stairs." If you build up/down stairs one on top of the next, they aren't stairs they are a ladder. If you build them as an up stair, then a down stair, then move over one square and repeat, you have a normal stairway, and won't get this problem.

Do you have your stairs right next to your barracks, where squads train? If so, dwarves will dodge into the open stairwell and fall.

Not quite a ladder, but more a spiral staircase within a single tile, and it probably has a supporting pillar in the middle, since staircases are self supporting.

I never usually worry about cave adaption, but I've heard people recommend revealing the top stair to the light (then reseal). The idea being that your entire central staircase is lit (because the light just finds a way to the next tile because its open). I don't really know if this works, I don't deal with cave adaption, but I've heard the advice. If this were the case, I would definitely say its cave adaption.

Dwarves have always tumbled down to the bottom of a stair case if they fall, for any reason. It just doesn't happen often in a normal fortress, so we don't care. Its not a bug, you've just got a lot of dwarves falling, and thats not good. Failed climbing attempts? Dodging onto a stair (there is a floor to stand on, should be fine)? Cave adaption triggering fainting? A drowsy syndrome got loose?...

A decent way to mitigate this would be to place floor hatches. A better solution is to strategically place floor hatches until this stops, and find the cause.

I think you should build the nobles quarters next to the stair.  Maybe the nursery as well.

Doesn't falling create a combat log? I've seen them when a...AHEM...worker on my doctor training system...falls. "The Useless Migrant hits the ground, the right arm takes most of the force, so on." Hasn't anybody else seen that?
