Hmm. Did you reload the game at any point during this fiasco?

In a recent fort that I've since abandoned, I had a dwarf who was being strangled for several hundred pages, even had their heart torn up, yet for several weeks they withstood punishment. Once I reloaded the game, however, they died instantly...

On another related note I once had an elite goblin bowman show up with a similar type of injury, though I couldn't observe any scars in his description
