"Well, let me just grab this priceless thread of rare candy from a pile across the fort to suture the incredibly minor wound up when there's pigtail fiber two steps away from me!"

Seriously, is there any way to stop my doctors from suturing with my candy thread instead of the worthless pigtails beyond forbidding all of it?

Don't think so. They'll go for the more expensive one because for some reason, Dwarves think sutures are some serious bling.

I was stuck at 14 dwarves with no extra migrants for my entire fort life until I got two 20-waves just a while ago. My militia needs armor and I didn't have furnace operators, but I had a master strand extractor. He makes strands out of my ~100 candy before I can smelt it, not to mention I can't figure out pumps for the life of me so I have to set a stockpile down in the caverns where my magma forge is.

In short, I have more strands than I know what to do with anyway, but I'd prefer if they weren't used to suture a torn finger.

In my fortresses I perform adamantine production in locked-off areas.  I make an area with craftsdwarf workshops and magma smelters, a stockpile that only takes raw adamantine, a supply of food and drink, and living and dining quarters for the workers.  When I'm ready to make wafers I send the workers in and lock the doors.  They don't get let out till all the adamantine has been made into wafers.

Admittedly it's a bit of an annoying workaround for a bug, but I think it makes thematic sense.  The production of adamantine is a dwarven secret, and takes place behind locked doors and armed guards.  You wouldn't want a human diplomat to wander through and learn how strands are extracted, do you?

Whenever a dwarf of mine is in hospital I just watch the docters and lock them into the hospital whenever they look for thread.

...

I imagine dwarven ninjas would do stuff mechanically, like setting up traps and poisoning food with their pus, etc.
