I didn't bother with soap for a long time, and never really saw any problems from it.  My dwarves did get infections fairly frequently, but it didn't seem to matter much.  I ended up losing a couple of soldiers to infection after a siege though, and decided that making a little soap was probably easier than training up a couple of meat shields to some decent level of skill.
