Fallout New Vegas with the Increased Wasteland Spawns mod. You don't know how many times i've died already playing a new melee focused character.

Going across lava on a gravel bridge with my hefty amount of diamonds, (7, to be exact) when I get to the other side, FOUR CREEPERS APPEAR FROM THREE DIFFERENT WELL LIT AREAS AND I FUCKING DIE IN THE LAVA.

In Dwarf Fortress, on my most successful fort Boldropes, once I breached HFS and the exact moment that happened: "A vile siege of darkness has arrived!"

Cue multiple facepalming at not having savescummed. I went out the dorfy way- sent my strong squad to the depths to contain the demons, and tried to erect a stalling path using hurriedly conscripted masons, then sent the other three squads to the surface to contain the siege. I left the fifth squad inside as general defense- some stationed nearish to the entrance as last line of defense, and some stationed near the stairway to HFS.

Goblin siege was broken, with gigantic casualties. The 'Diamond Gods' were down in the depths fighting a losing battle- all but the most powerful dwarves were swept aside in a few seconds. Iteb scored three kills before getting a melted head and falling unconscious- thankfully the demons left him alone while he was unconscious. Zefon scored no kills but multiple arm wounds. "Kurok" got no kills, no injuries beside a left arm that went necrotic from a sting.
I abandoned before Iteb and Zefon got killed- those two were my favourites and I wanted them to escape.

Minecraft: I was ambushed while outside by a creeper. It dropped me to one heart and ruined my burrow. I hid in the hastily-patched remains of my burrow until my health had regenerated enough for me to repair my burrow's entrance - by which time it was day. I crept out, repaired the burrow, got ambushed by another creeper and got dropped to half a heart. FUUUUUU-
Sprinted out to try and get back to the burrow's once again damaged entrance, regenerated my health very slowly because i'd gotten hungry, then when it was full again...

I descended to the depths of the chasm i'd colonised and promptly got shot to death by a skelly. RAGE.

Runescape: This is a historic one I remember. I was just about to do the final boss battle in the quest "Shadow of the Storm" and realised that I'd forgotten the crucial MacGuffin to defeat the boss with- I raged for a few seconds while Agrith-Naar (level 100) whittled my player (then level 76) down to 2/68hp and I hit my "EMERGENCY ESCAPE BUTTON" (cabbage-porting using my Explorer's Ring) and found myself in Falador on 1hp. So close. Healed, prepared again, walked the bloody ages to get back to the area I was in, and fought Agrith-Naar again. He demolished me in about four hits, bringing my character to 10hp / 68hp. I panicked and ran, then remembered that I had plenty of food. I ate, then played a war of attrition- Dragon Dagger P++ Special attack! 20-20! Ooh yeah. Run away and range with my crappy Maple Shortbow and crappy Iron Arrows. Yeah! 4, 4, 2, 0!

Then I ran out of food and figured "It's all or nothing now. Let's do this." Put my prayer on to protect myself from magic damage, charged in with Silverlight and got Agrith-Naar to very low hp. At which point for some reason I was hitting 0, 0 , 0, 0, 0. I kept hitting zeroes on Agrith until i got a lucky hit and defeated him.

I had seriously underestimated Agrith-Naar. I'd fought a level 172 Black Demon not long before that with very little trouble at all (of course, Black Demons only use melee so I could protect myself but Agrith used Magic and Melee.) RAGEEEEEEEEEEEE after I'd finished the quest at my stupidity.

Every time a Cultist spawns with a Heavy Missile Launcher, a weapon capable of One hit killing any of my soldiers. I play it safe; keeping my guys behind pillars or other cover in the hope the damn missile will hit that. Only works 10% of the time anyway, due to the damn missile having absolutely brilliant homing capability, and more often than not I lose one of my better soldiers to a bullshit weapon seemingly built to screw my guys over. Too powerful and expensive for me to use against Aliens or Cultists, but free for the Cultists, and just powerful enough to blast through my armour.


Xcom: Apocalypse. I'm dreading the goddamn Enzyme Launcher.

Runescape: This is a historic one, Agrith-Naar.
You must of been pretty unlucky, I Beat him with my lvl 60-something on the first attempt.

Yeah, i went in cocky, having only hours before soloed the 172 black demon for the Grand Tree quest with no effort needed. :P
That same character had no problem soloing against level 100 plus bosses in Dungeoneering until Shadow Forger-Ihlakhizan- once i got utterly annihilated in about a second because it acid-shot me three times in one hit (level 353 if i remember right- i was level 86)

Fallout Tactics on Ironman.

The last 3 missions have to be done at one go, with a single death resulting in having to go through them once again. (takes an hour at least)

On the second from the 3-chain levels, at the end, there are some narrow passages, that you have to go through.
At the other side there are fuckloads of hostile robots, including some giant ones that go into "melee".
These "melee" robots are as wide as the passageway...

...The wrecked ones (Once DEY BE DEAD!) can not be destroyed or walked upon. And there is no second passageway. Now imagine, if one died inside a passageway..

Having to reload cause of such BS is ball-breaking.

Observation : Fallout Tactics cd is a decent throwing object.

There was also this time in Battle for Wesnoth, where my firemage (70% chance to hit with a fireball) missed 43 attacks in a row.

And one can not forget Gothic 3 combat mechanics. Aka : You win by interrupting enemy animations, and they win by interrupting yours. Which translates to : If you hit the enemy ONCE and keep attacking, he will not retaliate, ever.
If you are attacked by a fast attacking foe (..wolf) and get hit once, you will not be able to do anything, as the "OMG IM HURT" animation makes you unable to act, and before it ends, you are hit again, and it resets.


Hmm.. That's it.

Mount and Blade:Warband-Mount and Musket mod

I hate it in multiplayer, when the lag is massive because there's no real good Asian server, I am a lousy shot, and when the enemy sneaks up on me, literally stabbing me in the back with their bayonet. 

Minecraft related:

Anything involving creepers really.  My first encounter with Endermen left me swearing in rage, shouting the last part of Pandarsenic's sig.  The skeletons are also my bane, I love swordfighting, but I cannot move in a circle properly or fast enough to kill them.  I even had a steal sword at the time, rare for me, and I still had a lot of difficulty keeping up with them snipers.

I was playing Dragon Ball Z Budokai Tenkaichi 3. I was in the world tournament with a customized Bardock.
On the 3rd round or so, metal Cooler defeated me.
A goddamn uncustomized metal Cooler.
He's a pretty underpowered character, and because he's an android character, HE CAN'T EVEN CHARGE KI.
My customized Bardock had even KING KAI'S TRAINING, which gives me more health.
I broke my controller in my rage. I don't know how the fuck that happened.

Civ 4 when I once attempted to take over France. They shared borders with me on one side and had halted my expansion, so I was mildly miffed. They then managed to take one of my towns due to secession and that broke it. I dissolved my alliance and prepared for war- little did I know France had been feinting and instead of the warriors and bowmen and catapults I expected, i suddenly saw musketeers. No matter, i was advancing fast. Then up out of nowhere my four other allies dissolve the alliance, ally with France and all mobilise against me. I had gotten France to the brink of surrender when I had to suddenly withdraw to shore up my northern front, by the time that had happened my armies were routed by the remaining French forces combined with the Dutch, and several others...
