I finally got an FB to appear, and I immediately sent all civilians to a predesignated area for protection, shortly afterwards setting up my first (and only) military squad to prepare for attack. About 5 minutes later (or less), the FB walked off my map. I turned off the alert and just tried to keep my civilians out of the caverns in general for a while. Suddenly, THE SAME FB THAT LEFT APPEARS IN MY STOCKPILE AREA. He didn't even walk up my mine shaft, he just sort of appeared there and I got a ton of messages of jobs being interrupted. How did this happen?! Help!

I have never seen a forgotten beast leave the map.  Are you sure it didn't just go into a part of the caverns you haven't explored?  That will cause it to vanish from the units list.

Ah, that might have been what happened. I sent in all of my military to attack it and so far 2 dwarves have died.

It died, taking down 5 dwarves before my military managed to kill it.

I have see them do this happen a number of times, though I have never seen them just magically appear inside a fortress. In one of my first embarks, one did manage to get the jump on me by swimming through a hidden cavern and emerging in the crater of the volcano at the heart of my outpost, but I had it coming.

Yes, though- FB, like regular animals, can leave the map and return. Or could, in 31.18.

I could of sworn I saw one swim off the edge of the map, return, depart again, and do this several times over the course of an hour.

Is there a well in your stockpile area without a grate?

Perhaps a well that connects to the caverns?

As far as I am aware, the only connection to the caverns (and the magma sea) is through my mine shaft. To get to my stockpile, something would have to climb up through the shaft, down my central hallway, through my meeting hall and down the stairs without meeting a dwarf.

Did you ever pierce the ceiling of a cavern anywhere as you were building your mine shaft? If so, a flying megabeast could get in there, if you haven't done anything to wall them off.

Cave-ins could be another explanation. Once, While I was building a suspended wooden platform it collapsed and punched a whole into the cavern below, clobbering a farmer.
