I'm not sure if this is a bug or a feature.. but I started a new fortress in a very large map with a volcano. Right as I started, the main crater collapsed with the usual "Section of your cavern has collapsed blah blah" -message.

Yeah, that happened to me, too. I'm guessing there were some free-floating floor-squares generated at some point.

It happens. Mostly because a volcano is generated after the terrain is. A volcano will be "cut into" the ground, with a blank space above the lava. If there was any hill above that, it will collapse.

Right now I've found two areas on my world that have:
a volcano,
an aquifer,
a complete lack of any height differences,
and lots of trees.
Basically flat watery forests. With volcanoes, right in the middle, not covered by anything. When I started a fort on one, a LARGE section of ground fell down into the caldera.

edit: now that I think about it - I don't get it. There were absolutely no hills shown on embark. Where the hell could the collapsed section have come from? Btw, I currently plan to just cover the thing with obsidian by pumping water from the aquifer. Bye-bye fire imps.

Volcanos aren't known for being very stable  ;)

The volcano I just started playing on has a hill over the volcano.  It covers most of the volcano in fact:  It's a giant overhang that I can walk up to the edge of and drop things off.  Wonder if I can make my dwarves dump buckets of water off that edge (it's too thin to channel water into).
