So, my Duchess went on break, leaving a huge queue of management jobs (legendary organizer) and unwoven thread (legendary weaver and clothier).

It's been a year or so since then.

Do nobles just not stop taking breaks?

They do stop.  It takes a very special procedure though.  It's a sort of, heat therapy... of sorts...

Is there another way? Believe it or not, I'm trying to do a fort WITHOUT intentionally murdering anyone...
Especially since she's got three legendary skills, and I like having masterworkX3 clothing.

That, and she's married to the biggest badass in the entire fort, artifact adamantine mail shirt and spear and masterwork adamantine everything else. Kinda be nice to not give him the slightest reason to tantrum.

If this guy scares you, he'll quickly sever someone's arm to give you a thumb to suck.

That's kinda the problem. If he severs the whole arm instead of just the thumb, then you don't get as much of the tangy blood taste.

To be honest, a noble's a noble. They work when they please. I'd just start training up other dwarves in the same profession and suck it up that you chose the wrong dwarf to be the duchess...

Just look at it this way, you're not complaining about her mandates, just her work ethic.

Try enforcing some medical treatment on her or can nobles be drafted? (I know that mayors can as a simple soldier)

Check her personality, see if it describes her as being incredibly lazy or prone to procrastination.

Lock her in a burrow. Don't let her out until she gets over it and gets back to work.

Or melt both of them at the same time.
