My guess would be by spamming a high-risk dangerous item over and over again. You can jack your wounds up as far as you want as long as you keep retrying the storylet. Also there are ways to increase your wounds once you're dead. I've been dead multiple times, been insane a few times, gone to the tomb colonies more times than I can count (quite a few times intentionally), and been to new newgate a few times as well (again, quite a few times intentionally). They are all inconvenient, but I tend to take the approach that as inconvenient as they are, they're just new areas to explore, and with new content being added constantly I've run in to new options in those areas on various occasions.

For what it's worth, going insane is by far the worst.

I had heard that they changed that so you only lose some of your dreams, not all of them. For precisely that reason. I'm still too nervous to try it out myself though.

The only other good way of obtaining them that I know of is to become a person of some importance and buy them in the bazaar side streets.
