I've hit this too. Granted, I have been eating kittens right and left so I wonder if the "unseen" vermin eating the food is plausible.

Also, I have had trouble getting them to stop "hunting for small creature" and eat food I get from merchants and start planting. I can't get them to plant the fields!!!

If it was vermin, should you see that somewhere? Is a kitchen required to exist for some background purpose even though they can eat plump helmets raw?

I've been having similar problems. My foodstores are full of prepared food and access to none is blocked. First I thought that the people who were starving were dwarves that were too poor to buy food for themselves. But now I saw an accomplished mechanic go in to "hunting vermin" -mode. He has about 3000 stashed away, so the cause is not lack of money.

It is not really an epidemic - more like a dozen dwarves or so starving each year - but it is still annoying, especially as I have no idea what is causing. I even lost one legendary metalsmith to starvation. It is starting to feel like a bug, as all the usual suspects seem to be innocent in this case.

edit - Ok, something fishy is obviously going on. My whole fortress guard and half of the royal guard just starved to death, even though their barracks was located just next to a full food storage with clear access. They all had a lot of money in their pockets, too. I have loads of prepared food and some meat and plump helmets . No, the dwarves are not prevented from going outside.

I had a problem like this one.
I had one dwarf inside while the rest froze, starved, dehydrated, were killed
and that one would refuse to eat any of the meat I had brought, even though the dwarf I eventually let in did. It was early year 1 on a turundra map, and I locked them outside. The food wouldnt appear on the kitchen screen, and it was in a stockpile.

Then they might have claimed it. I had the same problem when I played a "hermite" game (running a fortress single-handedly isn't that hard by the way). The dwarves outside took ownership of all the food. But my hermite survived by eating a mule, and the little bastards all died horrible deaths.. Mnjehehehe..
But they survived all winter. Talk about stubbornness!

The only problem I've had (And am currently having) is the dwarves refusing to stop Hunting for Vermin to eat some of the horse meat I set out for them.  Nothing quite of the magnitude of unexplainable self-starvation, but still rather strange, none-the-less.

Of course, it was my fault for not brewing all of the wheat I grew over the year...  Poor guys.  Now my Chef is dead...

i have had one odd problem, and its not a bug so much as an odd side effect of cheating.  i think the problem that some people are complaining about isnt a real problem, but a side effect of their cheating.  and they just neglected to mention the bit where they used a memory hack and expected it to work normally.

so my one odd problem is this:  i started a fort (repeated it several times, just to make sure) and gave my self extra points.  and anytime i pack huge quantities of food (like, 200 pieces of turtle or mule meat or something) the dwarfs dont actually eat it.

they move it from the waggons to the stock pile, and will even move it between different stockpiles (inside, outside, whatever) if i tell them to.  but they never eat it.  ever.

i cook it into meals, and then they eat it.  the fish or butcher new food, they eat the new uncooked food just fine.  its just the food that i bring at the start, after i have hacked more starting points in and brought extra food.

Bah, I have certainly never cheated. It would ruin all the fun in the game, unless turning otherwise non-trainable animals trainable by editing files counts. I wonder if in my case it might some sort of pathfinding bug, since even nobles are starving. That means it can't be caused by lack of money to buy food.

I ran into this issue when I set the "Dwarves stay indoors" option.  It seems like (not that I've tested it a lot) if you have any part of a stockpile outside when you set that, they sometimes have trouble eating.  I let them back out, wiped all my stockpiles, and made sure that they were all inside before resetting the "stay indoors" option, and it seemed to fix it.

Well, I don't have any stockpiles outside.

You don't need to cheat, just don't bring axes(thats why most of these errors are on trundra)
still, only my opinion....
but it seems to be from bringing too much food

I just tried my first glacier map and had a food issue when starting.  I brought quite a bit of meat, but never got around to cooking anything up.  Didn't bring any plump helmets, only spawn and other seeds.  I figured I shouldn't leave the food outside for long so I brought it in once I had a little main hall section dug out.  I was in the process of setting up my farm over the river when everyone started hunting for vermin.  I ended up abandoning, but for two reasons.

1)  I hadn't brought enough booze to start with.  :)

2)  Once they all started hunting, I knew I was screwed.

I had wondered why they didn't eat any of the meat...well, I looked at it in the barrels and it was listed as heavily worn from everything walking all over it.  I'm willing to bet this is why they wouldn't touch it.

Oh well...time for take #2!

Sure, unless everyone ends up hunting for vermin at about the same time. I'm not sure why I ignored the hunger arrows...but I guess I was thinking it had something to do with running out of booze right around that time.

Well, this happened to my fort today. Dwarves starved to death one by one.

Food Stores: 213
Meat none Seed 137
Fish none Drink none
Plant none Other 76

And I enabled cook option for seed.

-*-----
I found a poor mason died on the food barrels, oh...
