How do Mafia games try to avoid last second lynch votes? Is it that the day is usually extended if there's votes made at the very end?

No, it still happens, which is why people like to hammer if that's a thing in the game and otherwise have a few redundant votes so one scum can't suddenly swing the vote.

Also, changing the vote at the last second to serve an obvious scum interest looks really scummy and generally gets that person lynched the next day.
