Are you using any mods? Are you sure they're plump helms ('k' them)?

I did have a mod on it, forgot the name, but it only added reactions.

And yes, they are plump helms. I checked.

This sort of thing will happen all the time if you make dwarves or another race of underground farmers to live in human type sites on the surface, but other than that only duplicate raw errors would normally cause it. Seeing as how adding reactions that are usable in fortress mode demands changing entity_default.txt, you most likely have an extra copy of that file in your raws folder. Go into your raws folder, find the original one provided by DF, and delete it. If you're not sure, try to glean the size in kilobytes and delete the smaller one. Try right clicking on it and selecting the 'properties' option and you should be able to see it's size to the byte.

This will probably fix this error, and any others resulting from this mod. Unless you told it to not overwrite ANYTHING and the mod actually modified several files. Then i would suggest deleting all files related to the mod, reinstalling it, and if it asks you to overwrite anything say yes. The files are probably named Copy_of_X.txt, and the original X.txt's.
