a dusting of mud gives water
a small pile of mud gives stagnant water
a pile of mud gives water laced with mud

Huh, so laced with mud is worse than stagnant?

Yes, in terms of cleanness, though I don't know whether this causes more infections or something else.

For a long time playing df2010 I've struggled with infection. ...
What else can I do to prevent infection? ...

??? No one has mentioned yet that some dwarves are more prone to infection than others? I always consider how prone a dwarf is to infection before I draft him. As a matter of fact, when considering a dwarf for the military, I've been putting a higher weight on attributes such as strength, agility, disease resistance, focus, spatial sense, and kinetic sense over having combat skills such as Axedwarf, Crossbowdwarf, etc. Disease resistance, however, does not increase like the others mentioned above, so it is particularly important.

I can explain it to you, but I can't understand it for you.

Thing is, it doesnt matter how resistant to disease a dwarf is when the towering feathered golden tick farts on him, causing his internal organs to melt.
