I think dirt walls work, but not dirt back walls... I seem to recall reading that on a wiki...

Oh yeah. The sides don't matter. I actually remember killing off half a dozen NPCs with a hellstone house.

Do the same NPCs come back if they die? I hate getting the nurse when I want a merchant and feel lazy

Yes they do come back if you kill them. See also: guide deathtraps in previous versions.
Also, I thought that the background walls and floor could be dirt, but not the sides/ceiling. Guess you learn something new every day.
PS: On the topic of rooms, they have to be 10 blocks x 6 blocks.

All spears are 'findable', IIRC.

Spears in random chests, tridents in underwater chests, demon lances in underworld.
