I'd argue for three effective tiers:

Non-PvP

Molten (fixed)
Necro, Molten (current), Cobalt (current), Cobalt (w/out attack speed bonus)
Shadowscale, Meteor

PvP

Necro, Cobalt (current)
Molten (fixed), Cobalt (w/out attack speed bonus), Molten (current)
Shadowscale, Meteor

Molten with the bonus defense would be insane; it's over a 33% bonus on an armour that already makes almost everything trivial outside of PvP. Within PvP DPS/move speed is far more important than defense as a lot of weapons'll kill you fast anyway.

(If there's one thing I can do, it's argue about stupid balancing issues in games where they don't matter)

Uhh... Taz? Cobalt has the exact same mana effects as meteor, but with stronger defense and the attack speed boost.

Well I guess this would also depend on what other gear your planning to bring to the table. Let us assume your opponent brags that nobody can beat him because he has full molten and a nix with more meteor shots then would ever be needed, tricky to counter, right? Well you decide to break out full cobalt, rocket boots and dynamite, then fly over the screen and make it rain sticks of red death. In this example, using cobalt or meteor is much better than molten or necro, and a band of starpower is more useful to you than a cobalt shield. You just have to think about what set ability works with your gear.

Shadow + 4 * feral claw + cobalt shield + muramasa = blue blur of death.

Obviously, but the fact that there's always a counterpick doesn't mean that we shouldn't try to figure things out in a general sense. Besides, there's no reason why one should know what one's opponent's strategy is until you see each other (of course, not letting your opponent ever see you is a valid strategy in itself).

It's a shame that most mana-using equipment is so weak right now. The only real use for it is rocket boots.

Rocket boots and cobalt shield seem to be practically essential for high-end PvP, though, and bands of regeneration aren't anywhere near as useful as bands of starpower.

(You know, I haven't found a single feral claw.)

Well you can all know my strategy. HARPOONS!!! Just so that it is humiliating when I crush you with a weapon that does 15 damage. And when there is a pirate hat in game, I will incorporate that too... F34R MAH POONS!

Sorry, I have been having a lot of fun with that thing when I get a chance.

What if the fight's underground? What if my gear is, say, Hermes Boots, Cobalt Shield, 2x Regen Band and CiaB with a 'Nix and Necro armour (coincidentally close to my current setup)?  I'd be able to hit-and-run forever.

As I say, the only thing you're going to be doing with that extra mana is getting slightly more opportunity to fly intermittently.

If you hit and run, they can chase you for a few seconds then ignore you and go back to their objective. Regen bands regen health so slowly its not even worth it (since you could just gulp a single potion and basically heal to full instead of waiting for 100 seconds to heal the same amount with 2 regen bands).

Then he breaks out his rocket boots and flies around shooting you with his gun, since you don't regenerate mana while using mana, even though you have more mana regen items you would both run out at the same time. In addition dynamite being useless since he is flying as well and you die.

You regain mana while falling, so if you maintain a regular altitude while in the air, you go for a lot longer with mana regen gear.

Yeah, but harassment's the entire point of the strategy. It'd work very well if potions were arbitarily limited (which strikes me as a sensible idea in general).

If terrain isn't favorable, go somewhere where it is favorable (easier), or change the terrain (harder, considering how quickly you could get killed while trying to screw with the terrain by some of the super-powered weapons), or change your setup (I keep a bunch of different accessories in my inventory for swapping around).

The point of faster mana regen isn't to be able to fly forever, it's to be able to get back in the air again (or start casting spells again) much more quickly than normal, and stay up longer the next time you go up as a result of having regained more mana than someone else who the same amount of time with normal mana regen. You wouldn't fly up and toss dynamite, you'd fly up and shoot them repeatedly with the phoenix blaster. You can also use the ivy whip to grab onto buildings or hover in midair and stay in the air (though it makes you an easy shot if you stay still, you can also use it to fly between them rapidly while regaining mana; obviously you can't attack while doing it).

Seriously, though. Hit and run? Nix, handgun, flamarang, S.Cannon. Flamelash, for lulz. Actual escape doesn't seem to be a genuine option in most cases. In an actual pvp fight, you'd probably want balloon+cloud rather than boots, 'cause as nice as flying is and everything, it's not terribly agile.

What we really need is the ability to bind the hook/whip to a key and use them independent of weapons. That would make PvP battles a thing to behold.

Yes, if you just have the boots on, you're definitely not going to be able to rocket out of melee range of someone with a fiery greatsword, for instance (I've tried :P), but on the other hand, if they lack a cobalt shield, you can just thwack them with a flamarang repeatedly to throw them back. The balloon can be useful, but I wouldn't add the cloud: It's taking away a slot from something more useful. Rocket boots definitely beat it. A cobalt shield definitely beats it - hell, it's the most useful thing you could possibly bring. Knockback wins fights. I'd even take a band of regeneration over it, because CiaB interferes with the rocket boots (requires an extra jump keypress) and doesn't save you from falling damage in 1.0.2.

Edit: I also keep the whip on something like 3, so I can easily switch to it, grab something, and switch back to something else. Potions are on 1, sunfury on 2, etc. Most of the other stuff beyond 4 I have to click on to switch to.

If your quick with your keys, making a quick dodge is done with the grapple (Protip: Assign it to key 2) and then you make distance with the boots. Balloon can be helpful, but CiaB is likely to get you hit again. Shooting targets with the nix while flying can be tricky, so if your good at it, go ahead, but otherwise, I stick with explosives. If your opponent isn't very agile, you can regen mana in the time it takes them to catch up with you, so you can fly and bomb forever, resting while they feebly run behind. But necro armour, balloons, and to a level hermes boots put an end to this.

In PvP, nothing works better than bottled environmental dangers. Bring a few buckets of lava to a dungeon fight, and let your opponent try the hit&run while he's forced to grapple to the ceiling. A surprise faceful of magma at melee range can be quite a powerful weapon in other circumstances too.
