I'm honestly not sure how FOBs even work. When is an infiltration reported? When an alert starts? Is it when someone notices something? I don't get it.

As far as I know...
If there is an alert, you will be notified asap.  Like if you are on when the infiltration is happening, you'll get an in-game alert.  ... If you are connected to the online mode.

No alert: You won't get any kind of report, except what you've lost after the fact.

I'm guessing whatever the worms are causing the infection, cipher is cultivating them and using them to make the skulls.

Is the pre-symptomatic indicator of infection full, womanly pink lips?  Because literally everyone in the quarantine has them.

I'm trying to get codetalker now.

The skull snipers, Ocelot suggests setting up an ambush of your own but a C4 onlyt akes off like a fifth of their health, I remember the ones in the yellowcake truck taking multiple tank shells to the face and surviving.

Is it actually possible to quickly and stealthily take them out?  If it's something spoilery like a One Weird Trick or a special weakness don't tell me.  Maybe I need to upgrade my C4.

Get your own sniper rifle and shoot them.  Three hits with the basic M2000 will kill them.  When they notice, start rolling around and they can't hit you.  They can shoot the pilot of a supersonic jet but they can't hit a guy rolling around on the ground.

I played sniper vs sniper battle with them and won out.  Find em, mark em and shift around after they see you, so that you have a little more time to line up the shot.  You can find them cause of their laser sights.(Silenced non-lethal sniper.) 
It might be faster if you bring a stronger sniper rifle then I had brought for that encounter, but at the least, it doesn't raise any combat alerts... where one or two of em tries to jam a foreign object into your face.

I meant while I was doing an infiltration. It never said "[whatever] CP: Entering Alert Status" at all, to my knowledge, and the defender still showed up. I know it's possible to do a raid without being caught (in which case the infiltration will show up grey in the defender's FOB Menu) but iunno.

Oh that, from what I'm reading, it seems allies/supporters of the FOB can show up and defend, even if you are perfect stealthing it.

Mission 22, I believe. Not quite there myself, but I'm getting closer.

Mission 22 is the Mother Base defense mission, and it comes right after you start Mission 21. You get an Emergency Alert and Kaz whining at you that Mother Base is being attacked (because obviously Revolver fucking Ocelot is too old to do it himself?? Why) and you have to save teh base, boss.
After you complete 22 you get a forced FOB tutorial.
