I have a large central hall that is the hub of my fortress. Leading off this hub are several up/down staircases. Each staircase is made of 4 individual up/down stairs, like this:

I'm pretending they're impressive spiral stairs. They lead up and down to identical setups on the nearby floors.

Occasionally one of the stairways vanishes and is replaced by "rough marble floor." Like this:

I can re-construct the missing one no problem... but it still seems odd. What if my ONLY stairway to a floor vanished? Would the dwarves on that floor be trapped for eternity?

It's becoming a regular maintenance issue. Every now and then I go through and re-build a few stairways...

I have no idea.  As with other problems, any paused save that captures this moments before it happens would be very useful.  Otherwise, it's very hard to track things like this down.
