Finally die from incineration; yes, it can take that long if they survive the blood loss from the fat melting.

WWUD if he was stuck on the surface with his entire fortress, and everyone he knows, submerged in lava?

Given that he no longer cares about orders, he'd pull whichever lever has the higher overall value. Even in insanity, no true dwarf would ignore fine craftsdwarfship.

WWUD after pulling the lever and initiating the fortress self-destruction sequence, while still stark raving mad?
