I just got this bug, here's a recreation of what I did:
I had 1 old save from 173.38a (I think), and a new pocket world from .38b. I had a fortress in the old save, and an adventurer in the pocket plane. I created a third world, a pocket plane, and decided to play an Elf, put all my points into swordsmanship, except for 2 ranks in both shield and armor, and 1 rank in swimming. I wandered around until I found a druid, and talked to him, only asking about what service I could perform. I talked to nobody else. He asked me to kill a Goblin Priest, I think. Anyways, I wandered around found the towers, and wandered around because I couldn't find a way up the cliff. I jumped into a pool, but said to go to the space above the pool, and drowned to death. I decided to create a new world, and it spammed the world.

EDIT: A slightly modded world, all I did was change the tileset, add [Soil_Sand] to some of the soil levels, and changed some init options. I didn't use Propector, Foreman, or reveal.exe. I could give more information if needed. This is the first
