Also, don't mine to the edge of the map. I think goblins can come in there. That's the only thing I can think of that would cause whole squads of them to appear at the ends of old mine shafts, at least.

I didn't manage to get a screencap of it, but my VERY FIRST attempt at a fortress ended this way... before i could even take my wagon apart. I was litteraly sitting right outside a HUGE gobbo fortress. They sent what looked like the entire army of Mordor at my dinkie little party, and all I could think was:
