Last update: I resolved this problem myself after support from the ISP got me on the right path. Netstat was showing some distinctly strange results and the one that caught my eye was Freenet - I had been running as a Node unknowingly without knowing it for ages. Uninstalling eliminated my lag entirely and it hasn't recurred
