There's also self imposed difficulty. Like playing You Have To Burn The Rope without using any weapons.

I guess... Like playing Freedom Force on hard without flying... ever.

Also without any of the super breaker abilities like Possession.

Unhackable, no-backup permadeath.
Vicious player-driven economy that's difficult to impossible to break into (much less actually do well in).
Randomized start location, race, class, and attributes make it possible to start the game without the ability to win in any meaningful way. In fact, many die during chargen!
Unforgiving physics, including weather, personal needs like eating and sleeping, and very deadly combat mechanics.
Skill and attribute gain is extremely slow.
Vague endgame.

It's ... real life!

Beating YHTBTW. I think it can be done with ninja reflexes, but can't be sure, because I can never manage to just those shots.

Of the ones that I have played personally (and by played i mean more than just picked up and discarded shortly afterwards), I think eternal daughter is the hardest.

I can confirm that it is possible to play YHTBTR without using weapons and only killing the final boss.

As long as you play it cautious, its fairl simple, but if you go in recklessly it can quickly reach touhou levels of difficulty
