The conditions are set out in the title.  The games most enjoyable element is pushing the story forward with character actions, (and interactions) that are believable based upon their character and thought process.  Strife encouraged.

My vote, (although, I never finished the game): Ogre Battle 64 
Beyond good and evil would probably be my second pick.

I only played the first Fable, I never felt like I was really in the main characters shoes.  The guy never talked to anyone, so it felt like he wasn't interacting with the world around him.  Even when he was mercilessly bullying kids around town or having hot sex.

The first Homeworld. It was a very simple story, but the execution of it was just breathtaking. Partly what caught me off guard was the uniqueness of the atmosphere, the design and the polish.
Shadow of the Colossus was also quite good.

Oh man, I just remembered a good one!  Star Trek Klingon Academy.  Produced by interplay, so you know the gameplay is good.

I can't say it's all that immersive, but PlanetScape: Torment has an absolutely brilliant story.
