Each person contributing the same amount, or the sum of the patients' contributions to the same amount as the healthy person?

If it's the same as blood types, then it could just be that the guy who walked in is double O negative (and is thus a universal donor).  If it isn't, then... I dunno, lol.

It's more complicated than that. It depends on the HLA genes. A more or less "perfect match" would be 6 out of 6. IIRC this number is relative as you can actually match for more (and, for that matter, I'm not sure right now as to which HLAs does the x out of 6 system match. I think it studies the major complexes for MHC I and II), but those six are the most relevant ones and useful for clinical practice.

It's because being ruthlessly utilitarian is the easy way to get through most of these questions.  It doesn't necessarily mean you could actually murder someone in real life in a similar situation.

1 dies, the other 4 lives.  (If circumstances permit...)  One volunteer or eenie-meanie-minie-mo.

Seriously, you need ALL sorts of information to make a decision...  not just be given a small amount.  (At least in the case of being doctor.) 
That is, unless this has to be done 'now' and there is no other chance... (For some unknown reason that will not and cannot be explained.)

The dying patients' organs aren't suitable for transplant.
