The Puzzel quest games are what I play when I only want to use a mouse.

Most flash games.

As stated, X-com, but I think you have to type in the base name.

I also wanted to ask though, why wouldn't you want to use a keyboard? MOO2 probably has the best mouse/keyboard use of a game ever. Every button on screen was mapped to the first first letter of the button title. Planets? Hit P.

Chime can be fun-ish.
I think I played MUD TV with just the mouse.
Everyday Genius: SquareLogic is GREAT if you like Sudoku-type games and their derivatives (more+larger squares, weird rules with addition and < > signs, etc)

FPS games can be played with just a mouse with clever reassignment of buttons (though I suppose this'd depend on the game, and is inefficient in any case when your opponents are not under a similar limitation).

I know a guy who played TF2 when he had a broken arm by just using one mouse button for shooting, scroll wheel for weapon select, moving the mouse for aiming, and the other mouse button for moving forward.

Can be done in toribash but you need to use the keyboard if you wanna pull moves as fast as most people, just 3 keys though.

Me thinks all age of wonder games need no real keyboard use to be played properly, great turn based strategy RPG, might wanna look it up. I think heroes of might and magic might fall into that genre too.

Back in the day, I had a mouse which instead of having a mousewheel, had a kind of...seesaw lever-y thing.  You could hold mwheelup and mwheeldown.  Great for moving forwards and back.  Add mouse4 and mouse5 and you can strafe in any direction.  +mouse3 to for weapnext, and...you're in business.  At the very least you could play a medic for sure.

Noctis! I think a few of the commands can only be done with keyboard but in general it's all mouse.

Cortex Command can be played with just a controller, and it works beautifully. They even suggest a control scheme that I would definitely go with.
