I only started using a danger room in my most recent fortress, and my military was usually quite good. You don't need high ranks that much if they are well equipped. I had very few deaths and slaughtered most sieges without problems.

But I kind of like that they are now legendary fighters, that sounds way more awesome. :D
Besides, not all tiles in my barracks have spears on them. (but the positions from where the marksdwarfs practice have, most of them are hammerdwarfs by now...)

But what would be the best way to trigger the spikes? I use a pressure plate at the staircase, because a lever might produce a job for a dwarf who is far away.

Whatever you like, really. An automatic solution would be to have a two-tile hole with an odd number of units of water, and have the back-and-forth movement of the water rapidly trigger a pressure plate.

I am trying to make chickens lay bees as eggs. So far it only produces a single "Tame Small Creature" when a hen lays bees.
Honestly at the time, I didn't see what could go wrong with crowding 80 military Dwarves into a small room with a necromancer for the purpose of making bacon.

What's the best way to make sure your military stays equipped with their equipment?

What's happened I think is that some of the guys are not equipping fully before they go to the danger room.

Beyond making sure the physical equipment is available, what's the best way to have everything setup/laid out?

I have the uniforms set to Replace clothing, stay equipped while inactive and a proper barracks. Should I set the barracks to personal and squad? Just squad?

Lever Profile. Fixes that at the least, and potentially gives nobles a useful purpose.

I don't mind my nobles at the moment. :D
All they ever want is pig iron and mittens (duke) and drums (mayor).
They don't mind working and my duke is my best mason. No reason to stop him from doing what he does best.

I do a 3X1 danger room, with a trap that has 10 spears. Like this.

XXX
XRX
XDX
XTX
XDX

The top door is set to internal and then locked, so the dwarves don't try to stand on the weapon rack, thereby avoiding the trap tile. It's very small, so all my soldiers are friends. Way I figure it, if my military dwarves are dying off in droves anyways, the last few might as well go berserk, guaranteeing a bunch more kills. I can always lock them out of the fortress if they pose a threat.

Just designate a 1x1 burrow, put a ten-spike-trap in it, and order your squad to guard that burrow.

Space used: 2 (1 for burrow, one for lever)
Mechanisms used: 3
Training spears used: 10

I don't think it's possible to get much more efficient than that.

This is what I do, but I usually have some doors to help avoid pet skewers.

Am I the only one who makes my militia commander activate the danger room lever? (From a windowed observation room!)

I also don't worry much about pets getting caught in there. Free food.

I actually go with 1 spear/trap. why? kids waltz in and out of the room.
well if I have time I set up veteran barracks and have them train with 5 spears.

If the children cannot survive a room full of training spears, the children weren't meant to survive.

all the children are supposed to die?
No, only the ones who are:

A. Stupid enough to wander into a training room.
B. Not tough enough to survive the trip through the training room.
C. Being used as shields by the mother.

What's the best way to keep everyone equipped in as many pieces as possible?

A lot of clothing is also being left around the fortress and no one seems to be picking it up. What can I do with that stuff?

Another important question:

When I'm choosing my equip for my dwarves, and I'm assigning gloves, boots and stuff, do I need to assign 2 boots, 1 for each foot? 2 mittens, 2 gauntlets etc?

Assigning hand and footwear applies in pairs, if you have a pair, that is. Not sure what you would need to do to make them wear say, multiple chain shirts, however.
