Well, clearly the dwarves have become so badass that they believe the only way to be healed of injuries is to dodge and block training spears. That way, the dwarf's inner soldier will come out and he will be too preoccupied training in the danger room to care about his broken arm.

Seriously though, do you have a hospital zone set up (i, h)?

If that mace dorf got no personal bed, you have no hospital zone and the danger room is set as barrack that's quite likely the end result.
