Injured dwarf mysteriously decapitated in hospital

After recovering from a FB attack, I got a message that one of my weaponsmiths had been struck down. Turns out he was resting from some kind of injury in my hospital. While I was looking around I noticed another dwarf come along and haul away his severed head, which had flown several squares. Instead of apparently dying of his wounds in a hospital bed someone or something had cause his head to fly clean off.

I do have one ghost on the premises, but this is a "restless spirit" and as far as I know those don't actually kill your dwarves. The only other thing is that my chief medical dwarf was an animal caretaker I promoted in the early days of my fort with no real medical experience. My hospital is extremely well stocked, and I now have a couple suturers, bone doctors, etc. milling around.

What gives?

combat reports.  Hit r to check them and see if you can see what caused it.  Sometimes inexperienced doctors can do this (i remember hearing of this sort of thing happening before)

That's what cages and minecart shotguns are for!  We don't need to control them.  We just need to aim them.

Check your Justice screen, see if he was chosen for punishment for not producing a mandated slade bed or something.

Turns out he was beaten by one of my miners with a pickaxe for not making enough low boots, even though he wasn't a weaponsmith. The fact he was was injured in the battle in my magma forges just made him a convenient target, but he was conscious and just had his upper arm broken when the miner took his head off with a no-quality copper pick.

Not sure how the metalsmith was injured in the first place, although I'm sure the forgotton beast throwing fireballs at everybody had something to do with it.

So one of your miners walks into the hospital and slams his pick into the neck of a dwarf, resting because of injuries sustained while fighting a monstrosity from the depths of the earth.  He then yells something about boots before slipping away, leaving others to find  his corpse.

There are horror movies that don't begin that well.

In the future, be sure to construct chains, designate rooms from them, and toggle on justice. The crazy justice people will beat people if there are no chains and it tends to end as described.
