Imagine this:
The hammer breaks the skin, as a result of stretching it excessively. It them goes through bone, and is then stuck as a result of other bone around it, as well as muscle and intestines/etc.

It is possible, it depends on the hammer more than anything else.
Skin can be broken by blunt force, take people who are attacked in RL with weapons like Baseball Bats - The blood doesnt exactly just sweat its way out

Though even with a normal hammer you can lodge it firmly in a wound. Try getting yourself a framing hammer or such and a joint of meat. Swing the hammer into the meat, it can lodge in there pretty handily. Or it might bounce/glance off sideways, so be careful.

going by that a hammer and pick are the same.

Just instead of 2 points it has one and a solid part -- esentially the same in that the pick part was the important part.

These developed from the things that were at hand i.e. picks and backs of hammers -- which are essentially different picks.
