People fear rejection. I fear someone's reaction TOO rejection. Which is the hard part of running a forum game, telling someone "no, you can't do that", especially during character creation.

It's a bid harder in a serious game but people will normally be willing to co-operate if you explain your position. If you're the GM then it's up to you, not the player.

Pulled teeth/other major operations are pretty much guaranteed.

Current plan is to get shitfaced before going in, so as to avoid screaming in terror through the entire thing.

Don't get shitfaced, mate. Even if Novocaine and booze don't usually interact, which I doubt, there are still personal reactions to consider, plus the fact that a dentist may refuse to operate on a visibly drunk patient.

BFEL once specifically went to a different dentist because said dentist was supposed to put me to sleep for the procedure.
They didn't, and the gas they were using wore off halfway through.
Dentist kept going anyway.

So BFEL has learned that its probably not a smart thing to ask for anything from a dentist.

There's always ketamine.

Or you could just chug a few shots of hard liquor right before entering the dentists, followed by brushing your teeth or chewing gum or otherwise disguising the smell on your breath (or just drink Hennigans, I guess). Then the alcohol should have kicked in by the time the actual sedatives wear off.          
You should probably ask Google just what the actual effects of mixing booze and novacaine or whatever are. I'm guessing probably not good.
