Shouldn't it be "Dwarvin Awards"?

I propose that only people who perform acts of outstanding stupidity be nominated for the award. Like a multilegendary wrestler losing a fight to a goblin lasher because he wrestled off the gobbo's thong and tried to beat him to death with it.
This is the IDEA, I know all dwarfs are retarded, but we need EXTREME retardation, oh and if you want to enter for this year, 1 month left, need good story and/or screen-shot, for !FUN!

My example pales in comparison with most others, but...
I'm building down one very long stairway from the surface to the water in the first cavern (20+ levels or so). That's because I need to build a vertical axle going all the way down from my windmills. Therefore, north of that stairway, the tile is channeled down into an empty space. I did not think this would be a problem, even more so as the first lower half goes very smoothly. I assume no stupidity-induced accident will happen.
Mistake! Terrible mistake!
I soon get the message that Urist McBestminer has collided with an obstacle and died. What?
I zoom down. I see what used to be his corpse, a bunch of limbs and his head on top of it, and a bit further, one leg, in the caverns. Ugh, that's gonna take a while to clean up, thanks but no thanks.
This cretin dug down the floor underneath him, thinking apparently it was a brilliant idea, therefore falling more than 15 z-levels down.
He was my best miner, but as someone else brilliantly said, he deserved ever z-levels of that fall. Now to train my new miners into digging the stuff in the right order.

I would like to nominate myself. I had built a nice little wall around my cliff side entrance, with fortifications to let my marskdwarves shoot down any passing goblins/kobolds etc. Unfortunatly I forgot something important. Water pressure is the one that doesn't travel in diagnals, but all dwarves/animals and most importantly, goblin sieges, DO.

I wondered how they all managed to slip in, and use my walls against me.

Another instance was when my first titan arrived, it wasn't too scary, a giant ear wig. So I got my marksdwarves ready and set the lever for my gate to be pulled twice, in an attempt to lure it closer to them. The bridge then dropped and the titan got on it. Turns out bridges can't be drawn up when there is a titan in the way.

I nominate the miner who decided to' help' another miner who was digging a ten z-level wellbore about halfway through.  Not only did he fall five stories to his death, but his body couldn't be recovered from the reservoir.

I nominate my expedition leader.
Embark location has volcano and glacier, so somehow there were hanging pieces of land. So just after pressing SPACE you get a cave-in.
So, first of all, that sucker were standing an the edge of volcano, and dust hit him, and he falls to volcano. I had to reload ten times before he fell to another side.
Then I wanted to melt some ice, to make farming. Magma tank was dug, I broke to volcano tube and unsuspended half-build wall that was to seal magma. That idiot came and built that wall from inside. Few second after he was burned.
Reloaded.
Again, trying to avoid cave-in, reloaded a few times. Finally he survived.
Then I repeated melting design, but used door instead of wall. I was completely sure that even such idiot can survive.
He came to carve fortifications and volcano's tube, and then you know what?

HE DESIDED TO TAKE A SLEEP. Just a few steps from lava flowing from hole.
- Oh, I'm soo sleepy... I'll lay here for a bit... Ouch! It burns! Aaaa!!! Help meeeeee!!!

He left a track of blood and died near main starwell from blood loss.
*facepalm* That guy surely has bad luck.

I wanted to remove the floors i had built to span a ~10zlevel river canyon, and I had hollowed out the inside to make a canyon fort. The idiots(me partially included) decide to remove the floor BY STANDING ON TOP OF IT. AND BREAKING THE BRIDGE UNDER IT. AND FALLING 10ZLEVELS INTO THE RIVER AND DYING ON IMPACT (actually, 3 fell and 2 died. The 3rd only broke his hand and leg and, later, when I went to check on him, I couldn't find him at the bottom of the river. I was all 'WTF?!' and went to the 'u' menu. He was upsairs, sleeping. Turns out he SWAM THROUGH THE RIVER, PAST THE ALLIGATORS AND HARMLESS MINNOWS(genesis mod) AND WENT UPSTAIRS TO TAKE A NAP. He's just dwarfier than the others, I guess.

All the many babies who head to the bottom of the cistern to "check it out" and wind up drowned when I fill it. "Babywater" is the term.

I got one. When showing my buddy how to play DF, I embarked on a map that froze during the winter. No biggy, I'll just make a cistern, which I did. After completion the miner decides to clean up a little, and nearly ends up drowning. I believe this is the dwarf who, after being given explicit instructions to pick up ammo for his crossbow, decides instead to hammer the unicorn with it, with all intentions of causing the valuable meat source to jump into the volcano. That dumb bastard is still in a small box for that.
